Calibrate is available to Compass Rose Health Plan members and their dependents who meet all of the following requirements:

• Are 18 years of age or older 
• Are enrolled in the Compass Rose Health Plan
• Are currently taking GLP-1 medications for weight loss
• Have a body mass index (BMI) of: 30 or higher, or 27 or higher with at least one related condition; prediabetes, high cholesterol, high blood pressure, cardiovascular disease, nonalcoholic fatty liver disease, obstructive sleep apnea, or polycystic ovary syndrome (PCOS)
• Are not pregnant, breastfeeding, or planning to become pregnant within the next two years 
• Do not have any of the following conditions: active substance use disorder, active eating disorder, cancer, recent bariatric surgery, active gallbladder disease, pancreatitis within the past six months, history of medullary thyroid cancer
• Live in the United States

Medication costs vary based on your health plan. Check your plan benefits for details about coverage, copays, and out-of-pocket costs.

Calibrate clinicians may prescribe GLP-1 medications as part of your treatment plan. These may include semaglutide, tirzepatide, liraglutide, or dulaglutide.

These medications are also known by brand names such as Wegovy®, Zepbound®, and Saxenda®.

GLP-1 medications help regulate appetite and digestion. They can help you feel full sooner, reduce hunger, and support blood sugar control.

To stay enrolled in Calibrate, you must actively participate in the program.

You are expected to:

• Track your progress: Record your weight and daily habits in the app 
• Complete curriculum lessons: Review and follow program content 
• Attend coaching sessions: Meet regularly with your Coach 
• Follow medical guidance: Attend clinician visits and complete labs as requested 

If you do not actively participate, you may lose access to the program and medications. You may not be able to re-enroll for 90 days.

To upload lab results in the app:

1. Open the Calibrate app 
2. Tap Support 
3. Select Message Support 
4. Choose Labs as the category 
5. Enter your message 
6. Select Send an Attachment 
7. Submit your message and upload your file 

Make sure your lab results include:

• Your full name 
• Your date of birth 
• The date the labs were collected

Lab work helps your clinician understand your current health and create a safe treatment plan.

You will receive a lab order by email within two business days after you sign up.

Lab costs are not included in the program and depend on your insurance coverage.

Yes. A smartphone is required to use the Calibrate app, where you will access your program, track progress, and communicate with your care team.

You can also view some program content on a desktop computer.

Within 3 months before your visit:

• Comprehensive Metabolic Panel 

Within 6 months before your visit:

• Hemoglobin A1c 
• Lipid panel 
• Thyroid-stimulating hormone (TSH) 

Optional but recommended:

• Fasting insulin 
• C-reactive protein (CRP) 

If Calibrate generates your lab order, it will include all required tests, including a fasting insulin test and a C-reactive protein (CRP) test.

If you plan to upload your own lab results, make sure they meet all required test criteria. If any required tests are missing, you must complete them before scheduling your appointment with a clinician.

Your lab results must include the following on each page:

• Full name 
• Date of birth 
• Date of collection 

This information is required to verify your results.

Do not visit a lab without a valid lab order from Calibrate. To request a lab order, send a message through the Support Center in the Calibrate app.

Clinicians use your most recent lab results to create a safe and appropriate treatment plan.
