Dosing Information for GLP-1 / Combined GLP-1 Medications

Mounjaro is an injectable medication that is taken once weekly and is dispensed in pens with color-coded labels corresponding to the dose they should administer. There are six Mounjaro doses: Gray label pens dispense 2.5 mg, purple label pens dispense 5 mg, green label pens dispense 7.5 mg, pink label pens dispense 10 mg, blue label pens dispense 12.5 mg, and orange label pens dispense 15 mg. Mounjaro doses increase every four weeks, as outlined below, up to the maximum Mounjaro dose of 15 mg per week but often the highest dose is not necessary to see meaningful results.

Please compare the dose you receive from the pharmacy with the dosing schedule below to ensure you have received the correct next dose. If you receive a dose that is different from the schedule below, please do not give yourself an injection and instead reach out to the Medical Team through the Support Center in your Calibrate app for instructions.

  • Start with Mounjaro 2.5 mg (gray label) once per week for four weeks 
  • After completing four weeks of Mounjaro 2.5 mg, increase to Mounjaro 5 mg (purple label) once per week 
  • After completing four weeks on Mounjaro 5 mg, increase to Mounjaro 7.5 mg (green label) once per week
  • After completing four weeks on Mounjaro 7.5 mg, increase to Mounjaro 10 mg (pink label) once per week and continue this dose.

There are higher doses of Mounjaro available, but you may achieve results without needing an increase to the maximum dose. After reaching the 10 mg dose, the Calibrate Medical Team will determine if an increase to a higher dose is recommended. If a higher dose is recommended, the dose increase schedule continues as noted below:

  • After completing therapy with Mounjaro 10 mg, increase to Mounjaro 12.5 mg (blue label) once per week
  • After four weeks on Mounjaro 12.5 mg, increase to the maximum dose of Mounjaro 15 mg (orange label) once per week
  • Mounjaro can be taken with or without food at any time during the day. It helps to set a calendar reminder so you remember to take it on the correct day every week. 
  • The pen has glass parts. Handle it carefully. If you drop the pen on a hard surface, do not use it. Use a new pen for your injection.

Ozempic is an injectable medication that is taken once weekly. There are four Ozempic dosages that are administered through three different pens: 0.25 mg, 0.5 mg, 1 mg, and 2 mg. Ozempic doses increase every four to six weeks, as outlined below, up to the maximum Ozempic dose of 2 mg per week. Please note: The 0.25 mg and 0.5 mg doses are both administered through the red label pen for the normal titration schedule listed below; this means your first two Ozempic pens will have red labels. The 1 mg dose is administered through a pen with a blue label which will be your 3rd Ozempic pen; and the 2 mg dose is administered through a pen with a yellow label which will be your 4th Ozempic pen.

Please compare the dose you receive from the pharmacy with the dosing schedule below to ensure you have received the correct next dose. If you have received the wrong dose, please do not give yourself an injection but instead reach out to the Medical Team through the Support Center in your Calibrate app for instructions.

For your first ten weeks (0.25 mg and 0.5 mg doses), you will use red label pens.    

  • 1st pen (red label): Four doses at 0.25 mg (weeks 1-4) and two doses at 0.5 mg (weeks 5-6)  
  • 2nd pen (red label): Four doses at 0.5 mg (weeks 7-10)

For your next four weeks (1 mg dose), you will use blue label pens. 

  • 3rd pen (blue label): Four doses at 1 mg (weeks 11-14) 

For subsequent weeks, you will switch to the yellow label pen for your 2 mg dose 

4th pen (yellow label): Four doses at 2 mg (weeks 15+) 

  • Ozempic can be taken with or without food at any time during the day. It helps to set a calendar reminder so you remember to take it on the correct day every week. 

Rybelsus is an oral medication that is taken once per day. There are three Rybelsus doses: 3 mg, 7 mg, and 14 mg. Rybelsus doses increase every 30 days up to the maximum Rybelsus dose of 14 mg per day.

Please compare the dose you receive from the pharmacy with the dosing schedule below to ensure you have received the correct next dose. If you have received the wrong dose, please do not take the medication but instead reach out to the Medical Team through the Support Center in your Calibrate app for instructions.

  • Month 1 (days 1-30): take one 3 mg tablet each morning with up to four ounces of water. Do not have any food, other drink, oral medications, vitamins, or supplements until 30 minutes after taking your dose. 
  • Month 2 (days 31-60): take one 7 mg tablet daily in the same manner as the first month. 
  • Month 3 and after (days 61+): take one 14 mg tablet daily in the same manner as the first month.
  • It is very important to take Rybelsus properly as Rybelsus will be less effective if not taken according to specific instructions. 
  • Take Rybelsus in the morning AT LEAST 30 minutes before any food, beverage, or other medication.
  • Take Rybelsus with no more than four ounces of plain water.
  • Do not cut, crush, or chew the tablet.

Saxenda is an injectable medication that is taken once per day. There are five doses in the Saxenda pen: 0.6 mg, 1.2 mg, 1.8 mg, 2.4 mg, and 3 mg. Saxenda doses increase every week, as outlined below, up to the maximum Saxenda dose of 3 mg.

  • All Saxenda pens are blue
  • Watch this video for instructions on how to use the pen
  • Start with Saxenda 0.6 mg daily for the 1st week (days 1-7)
  • After completing 1 week of 0.6 mg, increase to Saxenda 1.2 mg daily for the 2nd week (days 8-14)
  • After completing 1 week of 1.2 mg, increase to Saxenda 1.8 mg daily for the 3rd week (days 15-21)
  • After completing 1 week of 1.8 mg, increase to Saxenda 2.4 mg daily for the 4th week (days 22-28)
  • After completing 1 week of 2.4 mg, increase to the maximum dose of Saxenda 3 mg daily for the 5th week and after (days 29+) 
  • Saxenda can be taken with or without food at any time during the day. It helps to set a regular time so you get in the habit of taking it and don’t miss a dose; choose the time that is most convenient for you.
  • One Saxenda pen can administer every dose level of Saxenda. You will need to turn the dose selector (dial) on the end of the pen to ensure you are administering the correct dose (please see this video for more information). 
  • In order to use the Saxenda pen, you also need to fill a separate prescription for pen needles (which we have sent for you). Make sure you receive both the medication and needle prescriptions.

Trulicity is an injectable medication that is taken once weekly. There are four doses for Trulicity: 0.75 mg, 1.5 mg, 3 mg, and 4.5 mg. Trulicity doses increase every four weeks, as outlined below, up to the maximum Trulicity dose of 4.5 mg per week.

  • 1st pen (yellow label): Four weekly doses at 0.75 mg per week (weeks 1 - 4)
  • 2nd pen (blue label):  Four weekly doses at 1.5 mg per week (weeks 5 - 8)
  • 3rd pen (purple label): Four weekly doses at 3 mg per week (weeks 9 - 12)
  • 4th and subsequent pens (green label): Weekly doses at 4.5 mg per week (weeks 13+)
  • Trulicity can be taken with or without food at any time during the day. It helps to set a calendar reminder so you remember to take it on the correct day every week. 
  • Each Trulicity pen administers one dose. At each dose level, you will receive four pre-filled Trulicity syringes.

Wegovy is an injectable medication that is taken once weekly. There are five Wegovy doses: 0.25 mg, 0.5 mg, 1 mg, 1.7 mg, and 2.4 mg. Wegovy doses increase every four weeks, as outlined below, up to the maximum Wegovy dose of 2.4 mg per week.

Please compare the dose you receive from the pharmacy with the dosing schedule below to ensure you have received the correct next dose. If you receive a dose that is different from the schedule below, please do not give yourself an injection and instead reach out to the Medical Team through the Support Center in your Calibrate app for instructions.

  • 1st pen (green label): Four weekly doses at 0.25 mg per week (weeks 1-4)
  • 2nd pen (purple label): Four weekly doses at 0.5 mg per week (weeks 5-8)
  • 3rd pen (brown label) Four weekly doses at 1 mg per week (weeks 9-12)
  • 4th pen (blue label) Four weekly doses at 1.7 mg per week (weeks 13-16)
  • 5th and subsequent pens (black label): weekly doses at 2.4 mg (weeks 17+)

Before starting Wegovy, there is some helpful information you should review.

  • Wegovy can be taken with or without food at any time during the day. It helps to set a calendar reminder so you remember to take it on the correct day every week. 
  • Each Wegovy pen administers one dose. At each dose level, you will receive four pre-filled Wegovy syringes.

Zepbound is an injectable medication that is taken once weekly and is dispensed in pens with color-coded labels corresponding to the dose they should administer. There are six Zepbound doses: Gray label pens dispense 2.5 mg, purple label pens dispense 5 mg, green label pens dispense 7.5 mg, pink label pens dispense 10 mg, blue label pens dispense 12.5 mg, and orange label pens dispense 15 mg. Zepbound doses increase every four weeks, as outlined below, up to the maximum Zepbound dose of 15 mg per week but often the highest dose is not necessary to see meaningful results.

  • After completing four weeks of Zepbound 2.5 mg, increase to Zepbound 5 mg (purple label) once per week 
  • After completing four weeks on Zepbound 5 mg, increase to Zepbound 7.5 mg (green label) once per week
  • After completing four weeks on Zepbound 7.5 mg, increase to Zepbound 10 mg (pink label) once per week and continue this dose

There are higher doses of Zepbound available, but you may achieve results without needing an increase to the maximum dose. After reaching the 10 mg dose, the Calibrate Medical Team will determine if an increase to a higher dose is recommended. If a higher dose is recommended, the dose increase schedule continues as noted below:

  • After completing therapy with Zepbound 10 mg, increase to Zepbound 12.5 mg (blue label) once per week
  • After four weeks on Zepbound 12.5 mg, increase to the maximum dose of Zepbound 15 mg (orange label) once per week

Before starting Zepbound, there is some helpful information you should review.

  • Mounjaro can be taken with or without food at any time during the day. It helps to set a calendar reminder so you remember to take it on the correct day every week. 
  • The pen has glass parts. Handle it carefully. If you drop the pen on a hard surface, do not use it. Use a new pen for your injection.

If you experience significant side effects as you increase the dose of your medication, you can go back to the previous dose and stay on that for several extra weeks before you try to increase again.  

Side effects typically go away within three to four weeks but if they are not improving, or are severe, stop the medication and reach out to the Medical Team through the Support Center in your Calibrate app. Guides to help with side effects can be found here.
