Who is eligible to join Calibrate?

Calibrate is available to Paychex members and their dependents for whom the following apply:

•     At least 18 years of age

•     Enrolled in an eligible Paychex medical plan option

•     Starting BMI of 35 or higher with at least one of the following comorbid metabolic conditions:

◦     Type 2 diabetes

◦     High cholesterol

◦     High blood pressure

◦     Cardiovascular disease

◦     Obstructive sleep apnea

•     Not currently pregnant, breastfeeding, or planning to become pregnant in the next two years

•     Does not have any of the following disqualifying conditions:

◦     Active substance abuse disorder

◦     Active eating disorder (anorexia, bulimia, or binge eating disorder)

◦     Active cancer

◦     Bariatric surgery within the past 18 months

◦     Active gallbladder disease

◦     History of pancreatitis within the past 6 months

◦     History of medullary thyroid cancer or MEN syndrome

•     Resides in the United States
